Forms

The following forms may be required for the completion of your program, confirm with your program coordinator when and which forms are necessary.

  • L1 – Registration for Licensure: All candidates fill out this form at beginning of program to fill this out, students will get instructions from the Educator Preparation Office
  • L2-PPC – Pre Practicum Contract: Contract between candidates, schools and program coordinators for pre practicum work
  • L2 – Practicum/Internship Contract: Contract between candidates, internship/school site, program coordinator for practicum experience
  • L3 – Candidate Assessment of Performance (CAP) Form: Require meetings and competencies between Candidate, Supervising Practitioner and Program Supervisor
  • L4 – Completion of License: Form confirming completed all necessary requirements to endorse license

Candidate Assessment of Performance (CAP)

The Massachusetts Candidate Assessment of Performance (CAP) is designed to assess the overall readiness of teacher candidates at the conclusion of their preparation experience.

  • CAP Observations: Examples of all observations (required and supplemental) during practicum experience
  • CAP Assessments: Examples of assessments (formative and summative) during practicum experience

MTEL

Depending on your license, you will be required to take the MTEL test of communication and Literacy as well as subject matter knowledge depending on your license(s) sought. Please consult your Program Coordinator for more information about your subject tests.

  • Workshops
    • Online and in-person workshop to familiarize yourself with MTEL and get proven strategies to be successful in MTEL exams
  • MTEL Deferral Form
    • If a candidate is unable to complete all the required MTELs prior to their Practicum this form must be completed with Program Coordinator permission

TK20

UMass uses an electronic portfolio (online assessment tool) to fully document a student’s key assessments, field experiences and student teaching experiences. Most students will need to purchase a TK20 license at the beginning of their academic journey
