All Initial Licensure programs have their particular programs of Study – but ALL candidates need to complete a series of L Forms (Licensure Forms) in order to process through the licensing process.

Pre-Practicum	L1	This form is SENT to ALL candidates BY THE ED-PREP OFFICE at the beginning of the program
	L2-PPC Pre-Practicum Contract	This form is for your PRE-PRACTICUM only

Practicum	L2 Practicum/Internship Placement Contract	Completion of this form secures your Practicum/internship placement and must be completed before you start your Practicum
	L3 Candidate Assessment of Performance (CAP) Form	Google Form – MAKE A COPY of this form to your own Google drive – so that the original does NOT change. Use THIS L3 LINK if you have trouble saving a copy of the L3 form.
	L4	Completed by Program Coordinators and Advisors within TK20
