This page houses most of the internal licensure, deferral, or waiver forms. Each form has particular instructions as to who should initiate them and what process to follow. Please complete then return to the Educator Preparation office for filing in candidate files.

Note: Each particular license has it’s own set of L forms – so please go to your licensure tab to ensure you use the correct forms.

The following forms are to be used for ALL programs for course substitution, SP wavering, or MTEL deferral. Each form below is filled on fully on line, see the instructions below to see how the forms should be filled in. Unless instructed otherwise – do not use the PDF provided below – as these are just examples FYI.

Course Waiver Form  (To be used when a particular area of content has been fulfilled outside of the approved program of study).

This form is to be started by the Program Coordinator. The Program Coordinator will be prompted to enter their name and email, as well as the candidate’s name and email on the PowerForm Signer Information. Once the Program Coordinator has entered the information about the course to be waived and the rationale of the waiver, and add their signature, the form will be sent to the candidate so that the candidate can enter the SPIRE ID and add signature to the waiver form. If there is any supporting document, such as a transcript from another institution, the candidate can upload them as an option. Program Coordinator needs to be sure to tell the candidate that this form will be in their inbox so as not to hold up the process. Once they complete their portion of the form, the completed form will be automatically forwarded to the Educator Preparation Office. The Program Coordinator and the candidate will be notified too.

Course Substitution Form (To be used when a candidate cannot take a particular course that is part of their approved program of study – so instead takes a different course as a substitute).

The Course Substitution Form is to be started by the Program Coordinator. They will be prompted to enter their name and email, as well as the candidate’s name and email on the PowerForm Signer Information. Once the program coordinator has entered the information about the substitute course and the rational of the substitution and add their signature, the form will be sent to the candidates. It is REQUIRED for the candidate to upload the syllabus of the course that is used as a substitution while completing their portion of the form. The completed form will be forwarded to the Educator Preparation Office at the end. The Program Coordinator and the candidate will be notified too.

Supervising Practitioner (SP) WAIVER FORM. Here is the PDF of this form so that you can see what you will need once you start the DocuSign.

This form is to be started by the Program Coordinator who should fill in the relevant role(s) names and emails. Once the form has been completed by those entered by the Program Coordinator, it will be automatically forwarded to the Educator Preparation Office.

DESE guidelines: 603 CMR 7.00 Supervising Practitioners should be an educator with an Initial or Professional MA license (in the subject and level that the candidate’s program), with at least three years of experience under the role, and a proficient or exemplary rating in the most recent evaluation, under whose immediate supervision the candidate for licensure completes their practicum.

If the Supervising Practitioner does not have all three requirements – please complete this waiver form. No program may waive ALL SP’s – this is only on a case-by-case basis.

MTEL Deferral Form. (To be used by candidates who have not successfully completed their MTELs at the various stages required by their program – must apply for admission into their Practicum semester – by completing this application.)

This form is to be started by the CANDIDATE. Before starting to fill out the form, The Teacher Candidate must discuss the purpose of the form with their Program Coordinator and understand the successful completion of MTEL is still required to get licensed. The candidate will be prompted to enter their name and email, as well as the program coordinator’s name and email. Once the candidate completes their portion of the form, it will be automatically forwarded to the program coordinator. Once the program coordinator completes their portion of the form, the completed form will be forwarded to the Educator Preparation Office.

WITHDRAWAL FORM: This form, initiated by the program coordinator, is to be used by teacher candidates who intend to withdraw from the Licensure Program, and will not be completing a MA DESE approved program of study. NB: The College of Education will not be able to endorse a withdrawn student for licensure through MA DESE.
